Health forma Boston, MA-based provider of science-based obesity treatments, has raised $38 million in its second round of funding.

The round was led by Sound Ventures, with participation from M13, SignalFire and Next View Ventures.

The company intends to use the funds to accelerate its work with corporate partners, support the recruitment of American Board of Obesity Medicine (ABOM) physicians to provide science-based obesity care, and invest in its telemedicine technology platform.

Led by CEO and Founder Evan Richardson, Form Health specializes in science-based obesity treatment. Its clinical treatment model consists of personalized one-on-one care from a dedicated ABOM-certified physician and CSOWM-certified dietitian who develop individualized treatment plans that incorporate nutrition, physical activity, and mindset change, and include a prescription for an FDA-approved medication when needed.

Since its founding in 2019, the company has served tens of thousands of patients across the U.S. through its telehealth model. Form is networked with most major insurance plans, including Medicare. Patients can work with Form through an employer benefits program or through individual enrollment.
